Is there a difference in connotation between the definite and indefinite articles, in this phrase?

I've done most of this, but I'm finding it too self-indulgent to bother finishing.

Re 19, I certainly am ok with the spelling in 19, being American myself, but I thought the usual practice was to have some sort of indicator in the clue.

Darla, I agree. I thought Gemelo’s use of “Alcatraz” last weekend to indicate a US treatment was rather clever.
It wasn’t clear if he meant the one with alligators!

Yes, unfortunately a reply to a specific comment in a thread doesn’t show up as such on this forum.

Re 19, I think it can be argued that the spelling in the solution is an accepted (albeit uncommon) British English alternative, so that an ‘American indicator’ is not necessary. Having said which, as mentioned above, I tripped myself up at first by attempting to type in the usual spelling, notwithstanding the fodder in the clue!

Well, I went back and finished it, but can't say I really enjoyed it. Favourite probably my FOI, 8a.

Struggling with this. About half complete but not sure if many of my answers are right. Or i have the second word of an answer but not the first. I have an answer for 2d but nothing that really fits any of the clue. A hint would be welcome for that and the first word of 20a.
