I parsed 20 as:
Definition "double", the first letter dropped (headed) from a 2,3 expression for "once".

Chris, I think it's questionable but I don't pass judgement on "fair".
To me, puzzles are a battle of wits between setter and solver. Sometimes I lose. Is it my dopiness or setter's unfairness? Who cares.

20a - I parsed it the same as Geeker. Chambers gives one definition of "headed" as ' to remove the head or to from'.
