Thank you hamishsoup - the problem is to do with certain of the entry fields, which give the error message

"Invalid email format (e.g. your_email@domain.com)"

when I enter something which isn't an e-mail address - so no problem with your fiendish creation - just with the technology! I have a vague idea this has happened before, and was corrected within a day or two. I'll be patient.

I think I have completed the grid - but I am completely lost on the dials. No ordering of the letters seem to make any sense to me! Help!

xwordfiend -- "Dial by dial in clue order" means all the 1s, then all the 2s, then all the 3s, then all the 4s... if you have correctly categorised the four kinds of letter, that will create a meaningful phrase

I did that and it made no sense. I guess I need to recheck my categorizations.

Robt - thanks! Have you successfully filled in the form?

Ah right -- to be fair I also initially mis-categorised a few clues but once a few words started to appear in the phrase I could reverse-engineer it to work out which I had got wrong!

Hamish -- I don't enter as I'm a Genius setter myself (Odo) and tester for two others, and so it seems wrong to partake in the prize element of things!

Aha! I have emailed the Editor to see if we can get this fixed :)
