1esk19, assuming you have the correct extra letters, there is an implied full stop after the second word, and and an implied comma (or and) after the third word. Then it all makes perfect sense

I've filled the grid, identified the thematic item and understood what to do in the final grid. But I have a problem with the extra letter from 15 across, which I have as I when surely it needs to be C. What have I done wrong?

The definition is the first three words, and then the wordplay is 2 (first extra), 3, 2

Thank you. I thought that the I came from court, and that led was a placement indicator rather than a definition.

It was 38 down that scuppered me. I have the constituent letters of the solution, apart from the unchecked, and only one word seems to fit - but buggered if I can justify it or identify the extra letter. Very frustrating!!

38d - def is last 3 words
"Mouth" is two letters and that is guarding "are" and "very good" with an extra letter in there

The 'very good' is a bit of a stretch in my view. Especially when that generates such a crucial extra letter.
