Thank you all for the help you have tried to give me. I'm afraid this one is beyond me. I have an idea what is needed but just can't parse most of the clues (particularly irritating is 3d....how & what? Nor can I work out the correct entry. Thanks again but this one is going to the great EV in the sky.

3d apparently = apt
bared loins = oin
put oin inside apt ....APOINT replace point by . in the grid

Xwordfan - yes it starts with comma. Think of a common expression for not wearing underwear.

I've been doing battle with this all week, I'm down to the last two clues to solve and I'm all out of ideas - please could somebody take pity on me and give me a shove in the right direction?

I'm fairly certain that both need part of the answer exchanging for a punctuation mark, I could be wrong though!

12a 6 letter word for design less the standard abbreviation for compliance => (5); grid entry (4) contains a symbol (2 letters).
18a look at the symbol used in the clue but not the more common term. To this 6 letter term, remove the last letter (not in the end) and anagram. Definition is uprising (heraldry).
