Ok, think I’ve finished now. I think the fifth word was what I thought it was, just a bit different from the other four. It’ll do me (I don’t submit).

Simond9x - sounds like you’re done. If you can see the fifth entry then it does appear in your grid - you just have to highlight three cells

Thanks, CD. I had that as the candidate for the 5th as soon as I got into the end game. My problem was I couldn't see it in the grid (as with the first four). The SE did have a possibility which I wasn't happy with. I decided that highlighting three cells was all that was required.

1. Three cells, full stop, correct? No other conditions? If that’s the case, I believe I can derive a sixth word using the same method, no?

2. I don’t understand the title.

Now that I'm done I can see I took a similar path to others with that last step. This one grew on me as I completed it. Some intricate parsing and a few very nice clues. Satisfying final grid! Thanks, Dimitry.

riversk - the title appears to be made up in a similar way to the five three letter words but with a different orientation.

I enjoyed the grid fill and working out the unclued entries. I like others was a bit uncertain about the three cells to highlight. There is one obvious word that drops out of the three longer entries. It does not appear in the grid in a straight line. It does appear in 3 cells that are contiguous but not in a straight line. It felt wrong.
Then the penny dropped. I should read the preamble more carefully and not make assumptions All fine now.
Nice one Dimitry.

Thanks mattrom (@ 40). I was counting "vital" as part of a seven-word definition, but now I see the two synonymous parts of the cryptic wordplay. Thanks again.

It’s the first half of a 10 letter word for an eye problem reversed giving the name of a famous hospital.
