I too am at a dead loss for clue 40 - a dunderhead knows how to get Elizabeth to reveal a portcullis
but for dabber11..
65 think of a well known port plus the end of trailer
77 look at the beginning of each word

I still have twelve to go, but the two that are driving me insane are # 14 - David Shepherd wickedly caged mountain beast (8) (anagram? David Shepherd the bishop?) and 99 - Second last clue from Exeter follows: leading dairy breed (5). Clues would be much appreciated., but I can't help with 40.

calliewag, the two clues you mention aren't anagrams but run throughs - the answers are contained in the clues. Hope that helps :)

No.99 - Begins with leading Dairy + Exeter but following the instructions in the first part of the question to give a breed of cattle.

One that is driving me mad is No.22 ...but no ham though for piles of Eastenders (6.5) something sheep?? Any clues appreciated.
