Lumen, it's good to give the clues...

12 A Relish acts going wrong, as far as stupidity (6)

NB All clues contain an extra word...

Relish is like chutney.... 'Going wrong' = anagram stuff. for the first part of the answer.

3 D Previous passage starred alto trumpet.

Previous.... means it's an old word. Put a trumpet after the usual Alto.

I can see what both answers must be, from the definitions, but don't see the wordplay for either. And the rest of the puzzle is clear, fair, clueing.

Hello! I don't suppose someone could point me towards this quotation? I don't have access to a copy of ODQ! I have the grid complete and author and the across and down provisions both seemingly more to do with GWTW than PL but I am stuck! Send help :)

Eider, you’re certainly at a disadvantage. If you google PL and the words which exemplify the two other quotations, you should find the missing one . Come back if you need more
