Help, please. Stuck on hopefully one only.

24D. Clue: Destroy, crush Scatter. 4 letters, Have the first letter R from 23A Sanatarium.
Then 26A clue is 'Acute or excruciating distress' for which I have "intense" of which the first e should make the 3rd letter. However, I can't solve it.
Any help appreciated.
