Actually, it wasn't that hard.
The theme was quite intriguing. Wonder about the back-story. (-;

The proper nouns, except for 18A, are common or well-known.

Yes, I too haven't parsed 40 A. The D will be the 'low grade' and the definition is fine, but as for the rest....
