Thanks, syzzie. I'm missing one letter in the regular grid (in 38A), and there are four other clues I can't fully parse. I thought at first that the outside letters would spell out "seen and not heard" like the P in those entries, but that certainly doesn't seem to work. I also don't get the reference to No. 16 in the instructions. Maltby isn't usually this obscure.

I'm guessing that there are shaded squares in the grid that spell "seen but not heard", but I still don't get the No. 16 thing.

Well done. It seems the shading didn't transfer to the copy .
I'll go to the library & check the original.
No. 16 is still a mystery.

Thanks to Aristo for sussing out the hidden letters.
I perused my copy with a magnifying glass & found there is shading that is barely visible.

If anyone is still interested, they are in (row, column):

1, 14

Apologies for the confusion. Anyone figure out what No. 16 refers to?

Thanks, Syzygy, and Aristo. Finished but with several unparsed. I was scratching my head about the letters until I saw your posts.

I wondered if 16a was something to do with using sign language, ' seen but not heard', but it does seem too obscure.
