Old tunic disused = the definition part of the clue and is an archaic word for old tunic.
two taps = ch (cold hot) and the rest is not an anagram as such, it is written in the clue.

...it's closely related, or maybe derived from the word migraine.

Got 37 now too...had found that word but again didn't look it up properly and only found the 'headache' definition.
