Thanks for all your help so far. I'm thinking of 'takeover' for 16d which just leaves 18a. I've now got ?O?K?T and am thinking 'docket' 'pocket'... Any suggestions to fit in with the clue 'Stock of money reduced by half after doctor's bill'.
