This has been given previously as SCALPEL. Apparently it's a sort of &lit (though not a very good one!)

To me it appears to be a straightforward cryptic definition,eg the typical hospital theatre shown on telly when the doctor calls for an instrument (here scalpel, to make a "cut during an operation") and it is "handed" to him/her.
