The surface read suggested a plural answer?

To me it appears to be a straightforward cryptic definition,eg the typical hospital theatre shown on telly when the doctor calls for an instrument (here scalpel, to make a "cut during an operation") and it is "handed" to him/her.

I'm with Ginge on this one. It is a typical Cyclops/Private Eye clue utilising politics and the health service, two of its favourite subjects.

Now I've read your reasoning Ginge I'm happy with the answer!
