42d....I've put 'Fall Down' as the answer; taking the initial letter 'F' from the answer to 41a....'SaFety'.

It's a poor bloody clue though!!

Its a poor clue but the only words that fit the 'known' letters are Fall Down or possibly Fell Down. Since the clue is in the present tense, the answer has to be Fall Down.
