Thanks malone and thanks kirky.
I did use Chambers Wizard to find that 7 letter anagram but it did not generate the answer.
I tried anagram solver and got it.

I am still working on 20a and 39a.
39a seems like an anagram but cannot work it out.

I have the three letter word and have penciled in the unclueds.

The disease at 28d is obvious but I cannot parse the clue.

For 39 A, are you remembering the 'with earl' part? That + the themed bit should make it fairly straightforward.

I didn't parse 28 D.

sunray, try "anagramming" 39a with ...
28d. a synonym of hide has been removed from upfront.

I'm puzzled by 38a. I know it's a thematic one. I've anagrammed the 'rejection' with the 3 letters and blank that I have in the grid but can't find anything. Can someone confirm that I've got the right letters? I have DI?S

Your I is wrong. cloverjo. The 'challenges' gives a 5-letter word, lacking energy = the usual crossword stuff. The thematic bit would come before your entry. Military leaders is the definition.
