Doctor's here for Dover sole 5,3,4
I have F??R?T, ?N?, L?S? So presume it must be First and last but I can't see why apart from a last being used to resole shoes??
Can someone explain?

All I can see is....

First and Last of Dover is - DR (Doctor)

First and last could also be interpreted as 'sole'

Can't quite get my head around the clue but a doctor is often present for births and deaths (first and last)

I can see that too Harry - but that leaves both 'Dover' and 'Sole' redundant.

sole = First and last
Doctor's here for Dover - D and R (or D over R?) from Dover Dr?

...D and R (or D'over'R?) from Dover = Dr
(first and last - one and only)
