All done, but I didn’t really understand the wordplay in 30D
Reading possibly about do, decline to join in

Graham9perkins, in 30d. the adjusted word is a surname, and 'do' is the first note in the scale.

A surprising amount of information being delivered from the adjustments to the clues. The co-ordinates didn’t work for me on latlong.net, which remained resolutely stuck on the equator; but GoogleEarth dropped me near enough to GZ to jog a memory. Quite the story.

I agree with all of the positive comments here. A very enjoyable puzzle so thank you, Karla.

Is the shape of the highlighted name significant?

Thanks mattrom, makes sense now. I had worked out the right adjustment but for some reason that particular person did not pop into my head until just now!

Enjoyed that, nicely done.

Also don't understand the significance of the shape of the name, if there is one, and agree 20d doesn't look right on the face of it.

Also struggled slightly with 43a - assume the correction goes in middle of penultimate word (?), but seems a bit clumsy if so

Yes, SH, the correction does go in the middle of the penultimate word in 43a. Check the entry for the corrected word in the BRB for an "inf." definition

Seems like an odd part of the body to choose though?

I was referring to "a bed (inf)"
