All done, though I'm darned if I can parse 40, after the omission.

I have a 4-sided shape (not 6) after 2 folds.

It was pleasing that the folding can be done and the entry submitted without mutilating the cut-out or detaching the Name and Address panel

I also have 4 sided shape after 2 folds. Thought this was very good, again. I also can’t figure out the parsing in 40, but for different reason. I see the venti, can’t see where the U comes from? Thanks.

I'm struggling to find the thematic song title from the letters in the central area - is it an anagram of the letters? So far I have ll, y, t, s, g, o, ?, i, t, e, ? Any tips appreciated

Izzet - no anagram - it reads across line by line. From the letters you give all seems ok apart from 7D (think you need alternative spelling so two letters appear in the blank rectangle....
