Fantastic news, Jolan, about your op. Hope all goes well from now on.

Thanks for another superb set of clues. Great fun.
AB

Here are my attempts-

1. Occasionally leave at six and arrive at ten, say (4,4,2,4)- From time to time?

From six (say) till ten (say) is from one time to another – from time to time = occasionally?

2. Slow movement of older commuters discommodes Afrikaner day centres (5)= Dumka?

The centres of OlDer commUters discoMmodes AfriKaner and dAy spell out “Dumka” in Chambers as “slow movement” (in music)?

3. Bank charges do not apply when Reichstag garden is renovated (8)- Gradient?

Anagram of “Reichstag garden” without “charges” is gradient = bank?

4 Bag containing the ultimate in synthetic heroin - Scag?

Bag as in “to bulge, to swell out, to sag” (in Chambers) with the last letter in synthetic –c is “Scag” – heroin?

5 Central features with high prominent bridges in Italy? (5,5)- Roman Noses?

Noses are central features. With high prominent bridges, and in Italy, they are Roman Noses?

Thanks again.

1 FROM TIME TO TIME?
2 DUMKA (middle letters). Dvorak's "Dumky" trio would be one of my "Desert Island" discs. It consists of six dumkas.
3 GRADIENT (subtractive anagram of reichstag garden - charges)
4 The best I can do is SACK (one of the slang names for heroin)
5 ROMAN NOSES (cryptic definition)

Thanks very much for a challenging set, jolan.

Great clues as always.
Here are my suggestions:
1. From time to time- Def: Occasionally ( Six and ten are both times)
2. Dumka- middle letters of older commuters discommodes afrikaner day (dumka musical term)
3. Gradient. Def: Bank- anagram of Reichstag garden (less charges)
4. Scag- def: Heroin- last letter of synthetic c in sag (bag)
5. Roman noses- play on words- central features on face- bridge as in bridge on nose

Having seen the answers, I cannot believe I actually saw Dumka and scag and hesitated to post as I could not parse. :(
