Sorry, I've just realised the graft is also medical .

Despite Ginge's hint I am none the wiser as to what needs to be highlighted, the curved line and what needs to be removed. At 1 across I have a radio pioneer- which could be completely wrong - but reading about him has not given me any pointers. As far as clue answers are concerned I have two queries. Is D the unchecked letter in 19 across? Can somebody please give me a hint to 21 across (Official member of SA rule) where I have NA?I?

Nazir means an official. But why Nazi for member of SA?

On 13a, I need a little more of a hint.

Thank you Sunray. I now have a full grid and know the route of the curved line. I now just need to work out the four letters which must be removed from the grid.

If you google the pioneer ... you should read somewhere what he wants removing

Thank you Xwordfan. I had been looking for something much less straightforward than that. The penny dropped after a much needed glass of red wine.

All you clever guys who've got the pioneer make me feel very stupid. I have a completed grid apart from the top row, and even found many of the possible letters therein. It would seem to start -EE (FEE, HEE or LEE) but none of those get me anywhere. LEE seems the most likely. I've tried googling the two words in cols 1 and 11 with the word pioneer, but all I get is people trying to sell me yachting kit. A gentle hint would be much appreciated.
