Chambers doesn’t specifically attribute 4a to the US and it suggests it is equally applicable as a term for a boyfriend.
I’ve tended to hear it only in the sense of partners “going .…..” rather than as a noun.

Amongst many others, I have no idea how you get the solution to 19d.

I’ve googled half the answers.
