It's one of the anagram ones. You'll kick yourself when you see it. Slang for psychiatrist.
I've finished but I haven't matched up all the bits, e.g. mutilate. It doesn't really matter but I hate loose ends.

I'm well over halfway through and about all I have are loose ends. Some clues seem to have switched words between them, including mutilate, which I feel relates to 16A.

Know what you mean about loose ends - it's taken me almost as long to tie-up the last few as to do the whole puzzle.

22down is also one of the anagram ones - I initially thought the definition for 22d was in 22ac but now I'm sure it's an anagram (of your word.)
