Still mulling over 13 Across.
Small numbers with horse disease as before (5) -ives
I know that 'vives' is a horse disease, but wondered what the 'as before' refers to - could it be the 5 in (5)?So maybe the answer should be 'fives'?

Chambers Dictionary gives FIVES (Shakesp) same as vives.

I reckon the answer is FIVES which was VIVES in Shakespeare's day - ie "as before"

How does the cryptic answer for XENICAL work please? (Clue was "Doctor buys this drug? Could be inexcusably")

INEXCUSABLY is an anagram of XENICAL and BUYS,
"Doctor" indicates the anagram.

I haven't completely worked out the clue apart from the ALPS being "up" so it must be SPLATS
