Thank you both for reply which obviously correct but can't see where 'caretaker' comes in myself. By the way, IXION was name of Blue Funnel ship which was known in Liverpool as the 'nine-to-one-on'.

In football they have a caretaker manager, which is to is an "acting" manager.

Is it me as no-one else is asking about 21 down? I am confused about the singular and plural in the clue - regular guys? (6)

Fit = HANDSOME, and I guess nurses could = HANDS, but what about the sanatorium?!
