Hello,

Can anyone explain how "note" element of the following clue fits the solution?

"Irritable little note about pre-med (5-8)" SHORT-TEMPERED

And I don't understand the significance of Tyneside in the solution to this clue:

"It turns for a stretch in Tyneside? Right (7)" SPANNER
