Taken me all week to do this one, just need help parsing three ( or l may have them just plain wrong)

I think it is Grimaced, but the only sense l can make of it is that distorting your face is a grimace.

1a) In America, check casualty's heart (6)
I think this is Ticker, which fits for heart, but the rest makes no sense to me

5d) Some film: shocking treatment making return again (2-5).
I have Re-emerge and guess some film is Ree(l), but why is Merge shocking treatment?
