Are you sure about the spelling? Famous5 gives Fastolf(f) while google.books gives Fastolff.

The crossword solver I use (TEA) only suggests Fastolfe as fitting.

"Fastolfe - citation - Complete Works of Shakespeare"

Fastolff Shakespeare gives 1020 Google hits.
Fastolfe Shakespeare gives 5710 Google hits.

Its up to you which you pick.
