I'm also tying myself in knots with 40. The only thing I can think of for 'knows how to' is 'can', and any other letters I want to put in don't fit with it anywhere as far as I can see. I have a weird possible answer that might be a portcullis, but that doesn't involve 'a', 'd', 'under', 'head' or 'dunderhead' so can't be right. Aargh!

I have come up with turkey - another word for dunderhead. Where the portcullis comes in I have no idea!
