Presumably the hospital department cut is EN(O) but how do you get ticed from 'beginning at the bottom'?

2. And dealer of such - extremely beautiful ones - concealed 7 perhaps. The answer to 7 is 'corgi 'and the answer to this clue is 'publisher'.

1. ENT (ear nose and throat dept) + iced

I think there might be a mistake in 2. PUSHER is dealer, around extremely BeautifuL, but 1s gives another s?
Corgi is a publisher of paperbacks.
