Jean, a doctor's nameplate is a SHINGLE, which makes your 'A' wrong.
And Terry, I think 'I have sinned' is PECCAVI, so that last 'A' may also be wrong.

Peccavi is the form(1st. person singular) of peccare usually seen in crosswords. So I expect you are right. But peccava is the singular imperfect tense of peccare so might be OK.
I got the above load of bollocks from one of those translation sites. I am sure you are right and I am wrong.
