The clue to 21 Down is: 4 by turning into 7 (2,3). The answer to 7 is 'lunatic' and I've finally worked out that the answer to 21d is my bad. But never heard the phrase. Any one know which dictionary the ST crossword uses so I can check if it it's in there!

It is a common US expression, getting increasingly common in the UK, and I'm surprised anyone watching any US television at all could have missed it.

The Times uses Collins, I believe, and it is in there.
