Simon lurches into hospital, then wanders out in total confusion (12)

When Odysseus came home to Ithaca he found his house in disorder; if he came back home again he would find...modern Greece, which certainly qualifies as omnishambles. Not overly subtle: shambles/madhouse after omni/all.

A difficult choice, I must say. My shortlist of favourites was, in the order they were posted: 5 (Rosalind), 7 (Bullfrog), 8 (Paul), 27 (BBM2 - a real doh! moment there) and 71 (Jamoza).

After much deliberation I plumped for Jamoza's late-ish entry, mainly because of its excellent smooth and amusing surface reading.

Query re warning "Make sure at least one person stays sober": If only one person is sober is it likely that everyone else can be trusted to make sure he stays that way?
