Ah, Bobbyf. This must be a cryptic crossie. I thought of the 'officer' answer but discarded it. I now see the 'cryptic' bit must apply to second = 'support' officer, eh?
Unfortunately, support officer doesn't fit with any of the down clues, so I've decided that it must be 'medical officer' - don't know why - and am finished! Off for lunch :)
