I have completed grid and have instruction.
But don't understand which 2 letters need inverting?
And then which 12 letters need highlighting?
Surely its not as simple as highlighting the first column!
Any hints appreciated

I'm as baffled as you.

Also what's the answer to 6d - Worthless and ultimately foolish streak (4)?

Streak = RACH (a bit tenuous but fits other clues)
Worthless = RAC(A)
Ultimately foolish = H

invert can mean "change the order" of some letters.
Look at the middle of 3 rows in your grid- you should see 3 words which are connected with 1d,20d 3letter word 4 letter word 5 letter word underneath each other.

Rach is defined as streak in Chambers but it is under the entry for Race - A white streak down an animal's face
