-c-e Im inclined to put acme though it could be acre. Reasoning?

it's acre but it isn't an anagram.
attention = care
first of all = a
a moved to top = acre.

nearly,care yes but first of all is the A so as it is a down clue A goes first,there is no anagram indicator
